If you can scan that image to a Bitmap file (or similar image), you may use FAB 3000 to convert the Image file to Gerber - plus generate Drill, Clean up, etc. Without an image file there's not much we can provide.
Note: For best quality, use at least 600 DPI Image (or higher) and generate a monochrome image (i.e. Black/White).
